A wisdom tooth — the third molar — is the last to erupt, usually between 15 and 25. When the jaw lacks space it becomes impacted, blocked by gum, bone or another tooth, which can cause pain, swelling, infection and deep gum pockets.

Crowding can occur when the wisdom tooth presses against neighboring teeth, causing damage to those permanent teeth. Additionally, because it is so difficult to clean around partially erupted or malpositioned teeth, inflammation leading to bone loss can occur, with possible infections.

Removal is planned from a panoramic X-ray or 3D CT that maps the nerve, then performed under IV sedation and local anesthetic. The tooth is eased out, sometimes in sections to protect the nerve, and the gum closed with sutures.
